Wellness Consultation (Intake)

Service Description

For those with health issues that want to see a wellness coach, herbalist, and holistic practitioner. Lily sees folks after an extensive intake form is filled out. The appointment lasts approximately 45 minutes and will be followed up with an email outlining a wellness plan we worked out together. If you require a custom made formula or supplementation, that can be provided and Lily will provide you a link to discounted supplements on her Fullscript dispensary and/or invoice you for a customer formulation.

Cancellation Policy

Cancellations with 48 hours notice can be refunded or rescheduled, client's choice. Cancellations without 48 hours notice can be rescheduled, but no refund will be issued. Chronic rescheduling (more than 3 times) will mean no consultation and no refund. Reserving a spot for someone means potentially denying a spot for someone else. Thanks for understanding that my time is precious!
